The Medical Group pledges to care for our patients who are carers. We aim to identify all our carers and do all we can to support their health and well-being. We have a carer display in all five of our surgeries and close links with Durham County Carers Support.

Patient Online will help you to take greater control of your health & wellbeing by increasing online access to services. If you wish to, you can now use the internet to book appointments with a GP, request repeat prescriptions for any medications you take regularly and also view your medications and allergies online. We are about to introduce an Electronic Prescription Service. This service will mean that we can send your prescription electronically to the Pharmacy you choose to get your medication from without the need for paper in most cases. The Electronic Prescription Service is reliable, secure and confidential.
If you would like to use the Electronic Prescription Service:
•You will need to ‘nominate’ a pharmacy to receive your prescriptions electronically. You can ask a member of staff in the pharmacy to arrange this. Practice Staff will also be able to arrange this for you after Tuesday 3rd June 2014.
•You can then continue to order your prescriptions in the usual way, but instead of a paper prescription being collected by you or your pharmacy, the prescription will be sent to them electronically.
•You should try to ensure that on most occasions you collect your prescriptions from your nominated Pharmacy. However, if you don’t wish to use your nominated Pharmacy for a particular prescription just make sure you let the surgery know when you request the prescription. For example if you wish to collect a prescription from the surgery reception, or a different pharmacy.
•Nomination is very flexible and can be cancelled at any time by asking a member of staff at the pharmacy or the surgery
If you feel you would like a chaperone present at your consultation then please inform your Doctor/Nurse who will be more than happy to arrange this for you.
Our practices provide management of care to all our patients who are ill or believe themselves to be ill, with conditions from which recovery is generally expected, for the duration of that condition, including relevant health promotion advice and referral as appropriate, reflecting patient choice wherever practicable. General management of patients who are terminally ill.
Our practices also provide management of care to those patients who suffer from a Chronic Disease, in consultation with the patient. This will also include the Care and management of Stroke or Transient Ischaemic Attack, Cancer Care, Hypothyroidism, Mental Health and Epilepsy.
These may be exchanged at reception. Please remember to bring the old batteries to exchange.
Our practices believe that prevention is better than cure and are pleased to offer a wide range of Health Promotion Clinics tailored to meet our patient’s needs.
Review and monitoring is offered to those patients who suffer from Asthma. The Clinic supports patients in the management of their condition.
We provide an anticoagulant therapy management service from all our surgery sites for patients aged 16 and over who have been initiated on warfarin therapy The service will minimise the potential side effects of warfarin by providing patients with regular monitoring to stabilise the International Normalised Ratio (INR) levels while continuing to maximize the effective benefits of such treatment.
These Clinics provide valuable inoculation against disease to babies and children under the age of five.
Full contraceptive services are available which include emergency contraception, giving advice about emergency contraception and where appropriate, the supplying or prescribing of emergency contraception.
Provides support to patients who suffer Respiratory Problems such as Chronic Bronchitis and Emphysema. The Clinic provides patients with regular review of their condition, measurement of respiratory flow, smoking advice and an assessment of treatment.
Review and monitoring is offered for those patients who suffer from Diabetes, both Insulin and Non-Insulin Dependant. The Clinic supports patients in the management of their condition together with advice on lifestyle, diet, weight and glucose testing.
Our Practice Nurses can provide information on contraception methods. The Practice does not offer coil fittings or implants; caps are only fitted at some of our surgeries. If you are a regular user of the pill, follow up is usually done by our practice nurses. Ladies aged 35 and over will be reassessed by the GP. Smokers will not be able to continue with the combined pill after the age of 35.
High Blood Pressure, Smoking, Diabetes, Gender and Raised Cholesterol are all risk factors in developing heart disease or strokes. We encourage everyone aged 45 and over to have their blood pressure checked every 5 years. Raised blood pressure has no symptoms making it difficult to confirm, please don’t be alarmed if we keep asking you back for review. Diabetes is very common; perhaps 4 in every 100 people suffer and it is more common if you are overweight.

Provides support to those patients who suffer with High Blood Pressure and Heart Problems. These Clinics provide support to patients in the management of their condition, regular monitoring and testing, advice, diet and healthy lifestyle
A specialist clinic is available to all patients within the practices for the removal of skin lesions and other minor operations. Please speak to your doctor.
If you have had a positive home or chemist test please book an appointment to see the midwife and take folic acid.
An experienced practice nurse who has undergone additional training to enable her to manage chronic illnesses such as diabetes, asthma and COPD. Our Specialist Practitioner is also able to prescribe a range of medicines.

All our Nurse Practitioners are able to prescribe a range of medicines. They are NOT able to issue fit for work certificates or repeat prescriptions. If you are unsure whether you should see the nurse practitioner or doctor, the receptionist will be able to advise you.
An experienced Registered General Nurse whose role is very varied and includes general treatment room duties as well as nurse led clinics in health promotion and chronic disease management.

Nursing Assistants are not trained nurses; however they have undergone extensive training and have a variety of skills. They work closely with other members of the nursing team.
A Registered General Nurse who has undergone additional training to enable her to manage and support patients with coronary heart disease and diabetes.
Community nurses are employed by the Primary Care Trust and work alongside the Medical Staff based at the practices.
The district nursing service provides a broad range of nursing care to housebound patients. This may be in their home, residential accommodation or where appropriate in the clinic setting. District nurses generally work in teams and are closely linked with The Medical Group Team, hospital staff and social services personnel. The overall aim of the service is to work in partnership with patients and carers towards maximum independence and self care.
A Registered General nurses who have undergone specialist training in public health. They work closely with GP’s, practice nurses and midwives and have close working links with social service departments, local authority education, housing, police and most voluntary organisations. Health Visitors are able to prescribe a limited range of medicines for minor injuries and illnesses.
The Midwives provide emotional and physical support to women during pregnancy. They work closely with the GP's managing the antenatal clinics within the practices. The midwives provide information to women in their care about all aspects of ante and postnatal care, within both the hospital and general practice setting.
If you have had this done before and not had any perforations or ear problems then syringing can be done by the nurse. Please use oil to soften the wax for 5-7 days, there is no need to see a doctor.
If you want to stop smoking, our staff will give you information about where to go for help.
Our nurses manage UTI's except in children, men and pregnancy. Please remember to bring a specimen of urine with you.
We have no magic answers. Groups such as Weight Watchers seem to do well. Remember it is a long term change in lifestyle, as well as eating habits, that is needed not a short term diet.
Some services are not covered by the NHS and a charge will be made in line with the BMA scale of fees.
